Background
The Centers for Medicare and Medicaid Services data report nearly 1 in 5 Medicare patients discharged from the hospital are readmitted within 30 days at a cost of over $26 billion per year. 35% of readmitted patients return in the first week post discharge.

Octavia was developed to fill the gaps in care team coordination. Octavia has been live, serving employees in 4 Dignity Health system hospitals on the California coast since April, 2015.
Problem
As the user base grew over the past 5 years, additional features were developed and incorporated into Octavia. Problems around onboarding and user experience start to surface.
